What to bring
Make your first visit easy.
Bringing a few things with you helps us hit the ground running.
- Photo ID
- Insurance card
- List of current medications and supplements
- Recent medical records you've collected (if available)
- Photos of the wound (if you've taken any)
If you also plan to bring any other physicians involved in your care — primary care, endocrinology, surgery, vascular, home-health — let us know so we can keep them in the loop with visit notes and updates from day one.
